TERMS AND CONDITIONS

You hereby agree and direct DoctorNow to provide you and any applicable family members 8 Antigen Tests by mail to your shipping address provided during registration subject to the following terms and conditions:

DoctorNow will continue to ship 8 tests per person per month until the Federal Public Health Emergency ends and/or your insurance no longer covers Covid-19 At Home Antigen Tests.

In order to cancel your participation in the Covid-19 At Home Antigen Test Kit program, you need to send an email to info@mydoctornow.net requesting cancellation, including the name of the individuals and their Date of Births.

In order to receive your Covid-19 At Home Antigen Test Kits the following conditions must be met:

  • Complete patient registration with accurate information
  • Provide active insurance information that DoctorNow can verify with your Health Plan
  • If uninsured, Medi-Cal Uninsured Application must be completed

Delays can occur due to unforeseen circumstances outside of our control, such as a strike, natural disaster, power outages, delivery delays, hardware or software malfunctions, and email errors.

It may take up to 6 weeks for Antigen Tests to be received.

If your insurance reimburses you directly for the cost of the tests, you are required, and agree, to remit that payment to DoctorNow. In the event that reimbursement checks sent directly to you by your provider remain uncashed (due to loss or other circumstances), you agree to call your health plan and direct them to re-send those uncashed reimbursement checks to DoctorNow. You also consent to permitting DoctorNow to contact your health plan directly and mandate that they re-route any reimbursement checks to DoctorNow’s preferred address of payment instead of sending them directly to you.

DoctorNow does not guarantee your tests will be from any specific manufacturer, regardless of tests shown in any marketing documents. DoctorNow will provide you with Covid-19 Antigen tests that are approved for at home use and hold an FDA EUA.

You must be enrolled in an eligible health plan. At-home COVID-19 tests may not be covered by all plans. Please check with your insurer to confirm as restrictions vary by insurer.

Please note that, while you may be eligible for up to eight tests, inventory may be limited.

DoctorNow is not accepting returns of at-home COVID-19 tests.

Concerns about defects with the product should be directed to the product manufacturer.
